Abstract (en)
This scientific research aims to determine both clinical and
dermoscopic characteristics of cutaneous metastases in patients with
primary cutaneous melanoma.
Methodology: The dissertation examines the association of
dermoscopic characteristics of cutaneous metastases with the
characteristics of primary melanoma, its histopathological features and
localization, as well as the clinical characteristics of melanoma patients
monitored after surgery at the Clinic for Dermatovenereology of the
University Clinical Center Niš, the Clinic for Oncology of the
University Clinical Center Niš, and the Clinic for Skin and Venereal
Diseases of the Military Medical Academy in Belgrade, in the period
from 2020 to 2024. Dermoscopic patterns of cutaneous metastases
were analyzed. Additionally, the types of structural patterns, additional
dermoscopic characteristics, and the dominant colour were determined.
The relationships between the examined parameters were examined
using statistical data processing.
Significance of the research: The majority of cutaneous melanoma
metastases had a homogeneous pattern (43.8%), followed by a vascular
(33.8%), saccular (12.5%), and polymorphic (10%) one. The frequency
of dermoscopic pattern types varied across different clinical types of
cutaneous melanoma metastases (flat and elevated) and between
pigmented and non-pigmented lesions. Flat cutaneous metastases
(macules) predominantly have a homogeneous pattern (72.7%), while
elevated cutaneous metastases (papules and nodules) most often have
a vascular pattern (39.7%); the homogeneous pattern is dominant
(65.3%) in pigmented cutaneous metastases, while the vascular pattern
prevails (87.5%) in non-pigmented ones. The type of dermoscopic
pattern varies according to the type of primary cutaneous melanoma.
In nodular melanoma, the prevailing patterns in its cutaneous
metastases are the homogeneous (46.3%) and vascular (36.6%) ones,
while in acral lentiginous melanoma, the vascular pattern of cutaneous
metastases is the most frequent (50%). The presence of additional
dermoscopic characteristics is significantly more common in elevated
than in flat cutaneous metastases. Non-pigmented cutaneous
metastases show a significantly higher frequency of blood vessels
(96.8%) compared with the pigmented ones (32.7%). Mitoses were
significantly more frequent in the histopathological findings of primary
melanoma in non-pigmented cutaneous metastases. In non-pigmented
melanoma metastases, the predominant colour is red (77.4%), while in
the pigmented ones, it is blue (51%) and brown (34.7%).
